Welcome to Emerson’s Junior-Senior High School Health Office! A full time Registered Nurse is available at Emerson Junior-Senior High School providing the following services:

 

Care of Students and Staff:

  • First Aid

  • Nursing assessment for illness

  • Nursing intervention for chronic health problems

  • Health counseling and referrals

  • Accident reports

  • Act as a medical liaison for students who have a temporary or chronic disability

State Mandated Screening:

  • Hearing

  • Vision

  • Height and weight

  • Blood pressure

  • Upon request of student/parent/teacher individual screening may be conducted

Maintenance of Student Health/Immunization Records:

  • Health records for each student maintained throughout students’ career including: medical history, immunizations, physical exams, and screening results

  • Ensure all students’ immunizations are complete and in compliance with NJ State requirements. In adherence with NJ State Law, students will be excluded from school for incomplete immunization records

Guidelines for School Attendance:

  • A student with a fever 99.8 or above, vomiting, or diarrhea should remain at home until 24 hours after symptoms are resolved or as instructed by a medical doctor  

  • Communicable diseases such as Strep Throat and Conjunctivitis (pink eye) require 24 hours of treatment before a student can return to school

  • Students with chickenpox must not return to school until all lesions are scabbed over (5-7 days). Communicable diseases will be monitored and reported to the Board of Health as necessary.

Medication Administration:

  • Oversee/administer medication as prescribed by private healthcare provider

  • Please note: New Jersey law prohibits the dispensing of medication (including over-the-counter medications) in a school without a written healthcare provider’s order and written parental consent

Environmental Health:

  • Identify environmentally sensitive students and staff

  • Collaborate with administration and the Board of Health to provide a safe school environment

  • Collaborate with staff/administration to effectively deal with potential crisis intervention and safety issues

  • Collaborate with staff/administration to identify students with special needs

Sports Physicals:

  • Assist the school physician with annual sports exams during the school year and in August

Important Information, Policies, and Necessary Forms for the School Year:

 

Emerson’s Board of Education and NJ State Law School Medication Policy:

  • Any student who requires medication, including over-the-counter medications, during the school day must have a written healthcare provider order for the medication and a parent/guardian authorization signature.

  • The healthcare provider's order must include the name of the medication, dosage, and time of administration. The medication must be in the original package or in a labeled prescription bottle.

  • Physician authorization for medication administration will remain in effect for one school year only. Any change in dosage requires a new healthcare provider note.

  • Please see attachments for the Medication Authorization Form. Please keep in mind, one medication per form.

Students with Asthma:

  • A parent/guardian and a student’s healthcare provider must complete an Asthma Treatment Plan form

  • A new plan must be filled out and signed for each new school year

  • The first page is to be completed by the healthcare provider and the second page is to be completed by the parent/guardian. Please specify if your child knows how to self-administer their inhaler medication in the designated section on the form.

Students with a Seizure Diagnosis:

  • A parent/guardian and a student’s healthcare provider must complete a Seizure Action Plan form

  • A new plan must be filled out for each new school year

  • Please see attachments for the Seizure Action Plan form

Students with Food Allergies:

  • A parent/guardian and a student’s healthcare provider must complete the Food Allergy Packet forms

  • New, signed instructions from the healthcare provider are needed for each new school year

  • Please provide your child’s EpiPen and any other necessary medication along with the healthcare provider’s orders

  • The medication must be in the original package or in a labeled prescription bottle. A healthcare provider’s authorization for medication administration will remain in effect for one school year only

Student Athletes and Physical Forms:

  • Per NJ State Law, all student athletes participating in a sport must have a physical each year

  • The student’s physical must be on the state’s mandated physical form which can be found under attachments on this site. If the physical is not on this specific form, it will not be accepted.

  • Our school physician must review and sign all completed physical forms. Those athletes that are not due to have a physical in the summer/fall, will be approved as their physicals become due.

  • To avoid holding up the physical paperwork process, please ensure all pages are filled out in their entirety. Please be sure to explain any yes answers in the explanation section accompanied by the year in which any occurrence/incident took place. Please also ensure the athlete and parent/guardian sign their name in the designated space. Physical forms will be returned if they are not completed correctly.

  • If you answer “yes” to any of the cardiac questions asked on the first page of the physical forms, i.e. your child had cardiac testing done for whatever reason in the past, you will need to attach a cardiologist note to the physical clearing your child to participate in their sport regardless of the testing findings. This is requested by our school physician and will ultimately ensure the utmost health and safety of our athletes.

  • Per NJ State Law, a health history update questionnaire form (different from the 4 physical pages) will also need to be completed prior to the start of each athletic season. Starting November 2018, this form will now be completed electronically. Parents must log onto their Genesis Parent Portal and fill out/submit this questionnaire prior to the first day of the season (including tryouts).

7th Graders

  • All 7th Graders are required to have a physical by their healthcare provider. Please provide the health office with a copy of the completed physical.
